Thank you for using the School of Dancing Wheels Fall Session 2023 Registration & Scholarship application online form. Completed student registration forms are due at or before the time of the student’s first Fall Session 2023 class. Fall Session 2023 consists of 12 weeks of classes from September 18-December 16, 2023. There are no classes the week of November 20-25, 2023.

This form will collect student personal identification and contact information; policy, media, and liability releases; class selection & payment options; and scholarship application information. School Policies

Low Enrollment/Cancellations: A class may be cancelled if enrollment does not meet its predetermined minimum. Classes must have at least three (3) students enrolled by the end of the third week of classes. Should this be the case, participants will be notified no later than the end of third week of the session. If the participant is unable to move to another class, the remainder of prepaid tuition will be refunded.

Participant Withdrawal: Tuition refunds will be given when a participant withdraws by the second day of their registered class(es). A processing fee may be applied to card repayments. If a withdrawal notice is submitted to the School Administrator after that date, the participant will receive tuition credit with the School of Dancing Wheels good for one year from the date of withdrawal.

Weather Related and Emergency Closing: In the event of a weather related or emergency closing, participants whose classes were cancelled will not be refunded, but may take any other class as a make-up class at any point during the remainder of the session (with written notice given to the School Administrator).

Monthly Payment Plans: Participants have the option to have tuition spread out into monthly payments with a commitment to the entire session. Monthly payments must be made by the end of the first week for each applicable month. A one-time payment plan fee of $10 will be applied.

Make-up Lessons: Should you wish to make-up a missed class either before or after an absence, you may attend any other offered class during the week with approval from the School Administrator. Requests must be submitted at least 48 hours before the desired class.

Private Lessons: Participants desiring private lessons must contact the School Administrator for scheduling and pricing.

Maximum Consecutive Absences: If a student is absent from three (3) consecutive classes without prior notice of no less than three (3) hours before class time, the student may be asked to not return to that enrolled class, at the discretion of the School’s administration. If a student is dismissed from a course after three (3) absences, any prepaid tuition will not be refunded. The participant will receive tuition credit with the School of Dancing Wheels good for one year from the date of withdrawal.

COVID-19 Health and Safety Practices:

The School of Dancing Wheels is dedicated to ensuring a healthy and safe environment for all class participants. Our policies will be updated as needed throughout the year.

Mask Policy:

The wearing of masks will be optional for all students, teachers, and parents/guardians. We will notify you if this policy changes to masks required. Please be considerate of everyone’s decision regarding masks. Bring your own mask. A limited number of disposable masks are available on an as needed basis.

Sanitization Policy:

Regular sanitization of all studios and equipment (ballet barres, floors, music systems, door handles, etc.) will occur. Wipes, hand sanitizer, and sinks with soap and running water are available.

Exposure Policy:

The School of Dancing Wheels follows local and national guidelines with regards to quarantining and isolating due to COVID-19 exposure or positive testing.

Regardless of vaccination status, we ask that you quarantine for a period of 5 days from the time of known exposure. You are encouraged to take class virtually during this time.
You may return to the studio after the 5-day period if you remain symptom free and have a negative test result. We ask that you wear a mask when you do return to the studio until 10 days from known exposure.
Continue to monitor symptoms and do not come to in-person classes if you are feeling unwell.

Wellness Policy:

Please do not come to the studio if you or anyone in your family or with whom you’ve had close contact feels unwell.
If a student, teacher, or parent/guardian at The School of Dancing Wheels tests positive for COVID-19, we will make efforts to immediately notify all individuals who may be considered a close contact so that they can follow recommended guidance.

I hereby agree to the above policies of the School of Dancing Wheels for the entirety of the Fall Session 2023.

Liability Waiver

I, being aware of my own/my student’s health and physical condition, and having knowledge that my/my student’s participation in any physical activity may be injurious, am acknowledging this release from liability for accidental injury or illness which may be incurred as a result of participating in this physical activity. I hereby assume all risks connected therewith and consent to my/my student’s participation in this program. In the event that medical care or treatment is required, I agree to be financially responsible for any costs incurred as a result. I acknowledge that I have carefully read this Liability Waiver and Release Form and fully understand that it is a release of liability. I agree to give up any right that I may otherwise have to bring a legal action against the Dancing Wheels Company & School and their affiliates for negligence or any other personal injury or property damage.

Media Release

I hereby grant permission to the Dancing Wheels Company & School to use photographs and/or video of me/my student taken at all Dancing Wheels Company & School classes, performances, and events in publications, news releases, online, and in other communications related to the mission of the organization. I understand that my/my student's image may be used in video recordings of virtual classes, and that these classes will remain the property of the Dancing Wheels Company & School henceforth. The Dancing Wheels Company & School may then use these recordings at any future time as they deem appropriate in providing future virtual programming or promotional materials.

Media Release Agreement(Required)
